Applicable for the following positions: District Directors, Regional Directors and At-Large Directors to the National Board of Directors. This form will also be used for consideration to serve as a member of a national committee.

**This is the nomination form for positions at the national level of the MU Alumni Association. This form must be completed in order for the individual to be considered by the nominating committee or the President of the Association.

All nominees must be current dues paying members of the Alumni Association. Board members must reside in the district or region they represent (if applicable). Verification that nominee is willing to serve must be provided.
